Maintenance Request Form - Office & Retail


    Please Complete The Form Below & Click "Submit" To Submit Your Maintenance Request(s). 

    List the hours of operation for your business. E.g. - 9:00 A.M. to 5:00 P.M.
    Terms & Conditions:
    Maintenance requests are handled in the order in which they are received. Contingent on workload, maintenance requests covered under your lease are typically completed within 48 business hours from the time they were received & processed. ​Emergencies such as fire & flooding have priority over all other less urgent requests. In the event of a fire, please call 911. For HVAC maintenance requests covered under your lease, our response time is typically 4 business hours from the time the request was received & processed depending on workload. 

    As a courtesy, TMT Properties can assist with maintenance requests not covered under your lease. However, we cannot guarantee an exact timeframe for these type of requests. Furthermore, you will be billed at $35/hour + materials for un-skilled labor (changing light bulbs, replacing ceiling tiles, door repairs, etc.) or $55/hour + materials for skilled labor (plumbing, electrical work, etc.) for these type of requests. Thus, you are welcome to contact your own vendors to assist with requests not covered under your lease.
